Yolo Continuum of Care (CoC) Provider Community
The Yolo County Homeless and Poverty Action Coalition (HPAC) is pleased to announce the Request for Proposals (RFP) for the Homeless Housing, Assistance and Prevention Program, Round 5 (HHAP 5). HHAP 5 is a $1 billion block grant program issued by the California Homeless Coordinating and Financing Council (HCFC). The exact amount of funds allocated to HPAC will be determined by the California Department of Housing and Community Development (HCD).
Round 5 is designed to build on regional coordination developed through previous rounds of HCFC Homeless Emergency Aid Program (HEAP), and HHAP funding. Round 5 funds should be used to continue to build a unified regional response to reduce and end homelessness informed by a best-practices framework focused on moving homeless individuals and families into permanent housing and supporting the efforts of those individuals and families to maintain their permanent housing.
To successfully reduce homelessness through this funding, HCFC also expects applicants to:
HCFC strongly encourages applicants to prioritize the use of HHAP funds to assist people experiencing literal homelessness move into safe, stable housing. HHAP funding should be housing-focused -- either funding permanent housing interventions directly or, if used for shelter or street outreach, have clear pathways to connect people to permanent housing options.
HPAC’s spending plan submitted to HCFC encourages aid to Yolo Counties Emergency Housing Services System, and includes funds for the following eligible services:

The Consolidated Plan (Plan) is a five-year plan, required by the US Department of Housing and Urban Development (HUD), which serves as a comprehensive strategy to address the needs of low- and moderate-income residents in the City of Davis (City). The Plan identifies community needs and provides a strategy to address those needs using Community Development Block Grant (CDBG) and Home Investment Partnerships Program (HOME) funds, as well as other resources.

Davis will conduct two public hearings to receive comments on the Consolidated Plan. The first hearing will be conducted by the Social Services Commission on Monday, March 17, 2025 at 7:00pm in Council Chambers.
The second hearing will be conducted by City Council on Tuesday, April 15, 2025 at 6:30pm in Council Chambers.

The U.S. Department of Housing and Urban Development (HUD) has published the Notice of Funding Opportunity for the FY 2024 and FY 2025 Continuum of Care Competition and the Renewal or Replacement of Youth Homeless Demonstration Program grants, authorizing homeless services organizations to apply for more than $3.6 billion in competitive funding to provide supportive services and housing programs for people experiencing homelessness.
This 2-year CoC Program NOFO is authorized by the Consolidated Appropriations Act, 2024 and establishes application submission deadlines for each fiscal year.
FY 2024 Application Submission Deadline: October 30, 2024
The NOFO revision adds $93,000,000 from an additional funding source to the list of funding sources that will be used to award CoC grants. The added funding will cover the costs of applying FY 2025 Fair Market Rents to renewal projects. There are no other changes to rating factors, policies, or eligibility for new projects. The Yolo County Health and Human Services Agency’s (HHSA) mission is to ensure Yolo County residents are healthy, safe and economically stable. Yolo County Homeless and Poverty Action Coalition (HPAC) provides leadership on homelessness and poverty in Yolo County. HHSA and HPAC have developed the 2023-26 Yolo County Plan to Address Homelessness. The Plan describes how the county will address homelessness through 2026. As part of this process, HHSA and HPAC convened a Committee of 14 stakeholders. Over six months, the Committee came up with tangible Action Plans for the Plan’s four goals. Please see attached files.

Continuum of Care Program
The Homeless Emergency Assistance and Rapid Transition to Housing Act of 2006 (HEARTH ACT) consolidated three separate homeless assistance programs into one single grant program.
Designated to help individuals and families experiencing homelessness.
The goal is long term stability.
Our 5 primary program components are Permanent Housing, Transitional Housing, Supportive Services, Homeless Management Information System (HMIS), and Homelessness prevention.
We plan to accomplish these components through the purpose as a Continuum of Care.

Sacramento Steps Forward is the Yolo County HMIS Administrator. Sacramento Steps Forward manages the database for Sacramento and Yolo Counties, granting access to the system, maintaining data quality, and providing regular reports to the US Department of Housing and Urban Development (HUD).
