Monitoring Homelessness Efforts

The Yolo County HPAC team is dedicated to ensuring transparent and effective monitoring of our initiatives aimed at addressing homelessness performance. Our performance and monitoring efforts are crucial to achieving our goal of ending homelessness and ensuring community support monitoring.

Performance and Monitoring Subcommittee

Roles and Responsibilities

The Performance and Monitoring Subcommittee evaluates recipients and subrecipients of all CoC-governed funds, including the Continuum of Care (CoC) Program and Emergency Solutions Grants (ESG) administered by HUD. The subcommittee provides recommendations to HPAC to enhance system performance and offer support and technical assistance to CoC members.

Responsibilities

Performance Targets: Establish performance targets tailored to specific populations and program types in consultation with recipients and sub-recipients.
Monitoring: Oversee the performance of recipients and subrecipients.
Evaluation: Evaluate program outcomes and take corrective action against poor performers.
Collaboration with Data Subcommittee: Assist in reviewing CoC-wide system performance reports, such as:
● System Performance Measures (SPM).
● Longitudinal Systems Analysis (LSA).
Consultation on ESG Programs: Collaborate with the Technical Subcommittee to consult with State and local ESG recipients on fund allocation plans, as well as the performance reporting and evaluation of ESG programs.
Funding Recommendations: Make recommendations to CoC regarding funding priorities to improve system performance.
Coordinated Entry System Evaluation: Serve as the Evaluation Entity for the Coordinated Entry system, if recommended by the Coordinated Entry Subcommittee and approved by the HPAC Voting Board.

Governing Documents

The Performance and Monitoring Subcommittee maintains primary responsibility for:

HPAC Performance Evaluation Policies and Procedures (to be developed).
● These may be incorporated into existing HPAC Policies and Procedures rather than existing as a standalone document.

Membership

The subcommittee should include representatives from:

● The Collaborative Applicant.
● Mainstream service providers.
● Recipients and subrecipients of HPAC-governed funds.
● Individuals with lived experience.

Participants with the following expertise are encouraged to join:

● Experience managing or evaluating the performance of direct service programs.
● Knowledge of HUD's CoC and ESG program regulations.
● Skills in data collection, analysis, and reporting.
● Familiarity with electronic databases and the data training needs of service delivery staff.
● Experience with evidence-based practices such as trauma-informed care, motivational interviewing, and housing-first models.
